Information Repository

This office records and stores the following city records:

Land records, mortgages, discharges of mortgages, liens, attachments, honorable discharges, certificates of ownership, plat maps and subdivisions (1731 to present)

Registrar of Vital Records

This office records, stores and provides certified copies of the following vital records:

Birth, marriage and death records (1731 to present)

We also store the vital records of the following towns:

Smithfield (1731 – 1871)
North Smithfield (1731 – 1871)
Lincoln (1731 – 1885)

To obtain a certified copy of a vital record, please download the appropriate application in the Information Center to the left. For fastest service, please fill out the downloaded application and bring it to our office on the first floor of City Hall (the address is on the top of the left-hand column on this page), along with either a check or money order (no cash accepted). You can also mail the completed application, with payment, to our office.

Clerk to City Council

This office attends all Council and committee meetings; and writes and maintains all minutes, communications, resolutions, proclamations, certificates, reports and agendas. Please click here to read Council Minutes, the City Charter or the City Code.


This office maintains files of all City licenses issued including:

Liquor, Automotive, Second Hand, Restaurant, General Services, General Merchandise, Permits, Entertainment, Amusement, Pool Tables, Applications, Advertising, Hearings Dog Licenses and those generated by the Animal Control Officer, Police Department, Code Enforcement, Tax Assessor and Tax Collector.

Probate Court

This office records court proceedings and issues certificates and decrees. To download probate forms, please see the Information Center to the left.

Other Services

This office also serves as a Notary Public and is custodian of the City’s seal and official documents.
