This office records, stores and provides certified copies of the following vital records:

To obtain a certified copy of a vital record, please download the appropriate application in the Information Center to the left. For fastest service, please fill out the downloaded application and bring it to our office on the first floor of City Hall (the address is on the top of the left-hand column on this page), along with either a check or money order (no cash accepted). You can also mail the completed application, with payment, to our office.
Clerk to City Council
This office attends all Council and committee meetings; and writes and maintains all minutes, communications, resolutions, proclamations, certificates, reports and agendas. Please click here to read Council Minutes, the City Charter or the City Code.
